Core Voyage Coaching & Payment Agreement

Prior to entering into a Core Voyage, LLC Coaching relationship, please read the following agreement carefully and indicate your understanding by signing below. If you have questions, consult your coach before signing. In addition to this agreement, you must also complete your intake packet prior to coaching. Contact us if you haven't already received your packet and we'll send it to you.

I understand that Core Voyage coaching is for people who are already basically successful, well adjusted, and emotionally healthy.

I understand that the role of the personal life coach is to assist me with improving the quality of my life.

I understand that Core Voyage coaching is based around a relationship with a coach that is designed to facilitate the establishment of long-range goals and short-term objectives and the achievement of those goals.

I understand that this coaching is comprehensive in that it deals with almost all areas of my life, including work, finances, health, education, relationships, and spiritual issues. I acknowledge that deciding on how to handle these issues and implementing my decisions remains my exclusive responsibility.

I agree to let the coach challenge me if they feel I am deceiving myself or on any areas they feel it is appropriate to do so, and I am prepared to step outside of my “comfort zone” with my coach’s support.

I agree to evaluate the life coaching process regularly and notify my coach immediately of any concerns. The coach can be honest and straightforward in asking questions and making requests. I understand that the power of the coaching relationship can be granted only by my permission. If I believe the coaching is not working as desired, I will communicate that belief and take action to return the power to the coaching relationship.

I understand that Core Voyage coaching does not treat mental disorders as defined specifically in the Diagnostic and Statistical Manual of Psychiatric Disorders. If I have anything in my past indicating that I have an unresolved and serious emotional or physical problem or a mental disorder, I certify that I am not using life coaching as a substitute for assistance from a mental health professional or a medical doctor.

I will not use life coaching as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, or substance abuse treatment.

If I am currently in therapy or under the care of a mental health professional, I will have consulted with that person regarding the advisability of my working with a life coach. Additionally, I will inform my coach of this relationship.

I understand that the confidentiality in the life coaching relationship is limited. Confidentiality will not apply to certain crimes that have either been committed or are planning to be committed. Such crimes may need to be reported to legal authorities. It is also permissible that certain topics discussed may be reviewed with other coaching professionals for training, professional support, and development purposes.

I will not use life coaching in lieu of professional medical advice, legal counsel, accounting assistance, business consultation, or spiritual guidance, and for each of these areas I understand I should consult the appropriate professionals. I acknowledge that I will not use Core Voyage coaching as a substitute for such professional guidance. I further acknowledge that all decisions on dealing with these issues lie exclusively with me.

I understand packages and sessions must be prepaid in advance. There is no promise of refund offered if I should terminate early. If an appointment needs to be rescheduled, I will notify the coach 24 hours prior to the appointment. If I fail to notify the coach 24 hours prior to the appointment, I will not receive a refund or credit for the time allotted for the appointment. If I desire to have more sessions with my coach, I will be responsible to purchase another package in advance to resume coaching.
