Participant Release of Liability Contract

For Experiential Activities And "Voyages" Retreat Participation

THIS IS A LEGAL DOCUMENT. PLEASE READ THIS DOCUMENT CAREFULLY BEFORE SIGNING.

Prior to entering into a Core Voyage, LLC Coaching relationship, please read the following agreement carefully and indicate your understanding by signing below. If you have questions, consult your coach before signing. In addition to this agreement, you must also complete your intake packet prior to coaching. Contact us if you haven't already received your packet and we'll send it to you.

I acknowlege that equine activities, SCUBA, snorkeling/swimming, nature-based experiential activities, travel, service activities, hiking, boating, or any other experiential activity that I will be participating in with the requested assistance of Core Voyage exposes me to many risks, hazards, and dangers. Some of the risks that may be present occur include, but are not limited to:
- hazards of physical exertion associated with the activity
- hiking in rugged wilderness terrain, far removed from the comforts and conveniences of civilization, like medical treatment, transportation and communication
- trail hazards that make hiking difficult, including steep slopes, rocks, and limbs in and over the trail, slippery rocks and footing, and holes and declivities
- injuries inflicted by animals, insects, reptiles, fish, and plants
- water hazards related to swimming, wading, snorkeling, SCUBA diving, capssized boat, or other floatation devices
- traveling in a vehicle not driven by me
- risks involved in decision-making, emotional discomfort, and motivational stress

I understand and accept that these risks expose me to, but are not limited to, the following consequences: death, serious neck and spinal injuries which may result in complete or partial paralysis, brain damage, serious injury to my musculoskeletal system, decompression sickness, drowning, and serious injury to other aspects of my general health and well being. I also understand that the risks in participating in the experiential activity include not only the foregoing physical injuries, but also impairment of my future abilities to earn a living, to engage in business, social and recreational activities, and generally to enjoy life.

I am voluntarily participating in activities and projects with Core Voyage, LLC with full knowledge of the risks, hazards and dangers involved.

These risks and dangers may be caused by my own action or inaction, the action or inaction of others participating in the Activities, the condition in which the Activities takes place, or any negligence of the "releasees" named below.

Understanding the risks mentioned above, and understanding that participation in the travel, activities, and projects of Core Voyage Experiential Activities may subject me to rigorous physical exertion, I hereby state that I and/or my family, including my minor children, who are fully capable of participating in Core Voyage Experiential Activities are physically fit to participate in any activity I participate in.

IN CONSIDERATION OF MY REQUEST TO PARTICIPATE IN THE EXPERIENTIAL ACTIVITY, AND AS A CONDITION OF THE RIGHT TO PARTICIPATE IN THE EXPERIENTIAL ACTIVITY, I PERSONALLY ASSUME ALL RISKS INCIDENT TO SUCH ACTIVITIES. I ALSO WAIVE, RELEASE AND FOREVER DISCHARGE CORE VOYAGE, LLC AND ANY OF ITS EMPLOYEES, CONTRACTED AGENTS, OR ASSISTANTS FROM ALL LIABILITIES, LOSSES, DAMAGES OR COSTS OF ANY NATURE THAT MAY ARISE IN CONNECTION WITH MY TRAVEL TO OR PARTICIPATION IN SUCH ACTIVITIES (INCLUDING RESCUE ACTIVITIES) ASSOCIATED WITH THE EXPERIENCE. I HEREBY AGREE NOT TO FILE SUIT AGAINST CORE VOYAGE, LLC OR ANY OF ITS EMPLOYEES. I AGREE TO INDEMNIFY AND HOLD CORE VOYAGE, LLC AND EMPLOYEES HARMLESS FROM ALL LIABILITIES, LOSSES, DAMAGES OR COSTS OF ANY NATURE THAT MAY ARISE IN CONNECTION WITH MY TRAVEL TO OR PARTICIPATION IN SUCH ACTIVITIES, INCLUDING RESCUE ACTIVITIES. THE TERMS OF THIS DOCUMENT SHALL BIND ME, MY HEIRS AND PERSONAL REPRESENTATIVES.

Prior to signing this document, I have had an adequate opportunity to read and understand it, have had an opportunity to ask questions about it, and any questions I have had have been answered to my satisfaction.
